AGREEMENT OF RELEASE AND WAIVER OF LIABILITY

It is your responsibility to inform the instructor of limitations before the session begins. If alcohol is present, It is your responsibility to inform the instructor of limitations regarding consuming alcohol before the session begins.

I understand that yoga includes physical movements as well as an opportunity for relaxation, stress re-education, and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will listen to my body, discontinue the activity, and ask for support from the instructor. I assume full responsibility for any and all damages that may be incurred through participation. I am assuming full responsibility for consuming alcohol, and by signing this, I am stating that I am of legal drinking age and 21 or older. By signing this, if I am under legal drinking age, I am stating that I will not consume alcohol at events facilitated by Yogapy LLC _Derdine Does Yoga or individuals affiliated with Yogapy LLC_Derdine Does Yoga.

Use Of Media
Yogapy LLC dba Derdine Does Yoga includes photos or videos of yoga activities on its website, educational materials, publications, and advertisements, including on social media. In such materials Yogapy LLC dba Derdine Does Yogawill not identify any individual student by name unless specifically and separately authorized. By signing this release form you are hereby granting permission to Yogapy LLC dba Derdine Does Yoga, its representatives and employees the right to take photographs of classes that may include you and authorize Yogapy LLC dba Derdine Does Yoga, its assigns and transferees to copyright, use and publish the same in print and/or electronically for any lawful purpose, including such purposes as publicity, illustration, advertising, and web content.
Please note: If you do not wish to grant Yogapy LLC dba Derdine Does Yoga permission to use photos that may include you on its website, educational materials, publications, and advertisements, kindly inform the instructor verbally during the event.

I have read and fully understand and agree to the above terms of this Agreement and Release of Waiver of Liability. I am signing this agreement voluntarily and recognize that my signature serves as a complete and unconditional release of all liability to the greatest extent allowed by law in the State of Florida and any state in the United States.
